Assistance with Daily Life

At Holy Cross Care Services,  we provide high-quality, individualised supports in your home and community. Our friendly and skilled support workers will aid you with various tasks so that you can live a convenient life.

Improved Health and Wellbeing

Holy Cross Care Services provides its participants access to capacity building for health and wellbeing. We offer a range of therapeutic services targets service of excellence to improve participants’ daily living skills which can be only achieved through person-centred intervention plans which are done thorough active listening, comprehensive assessments of relevant allied health services.

Our experienced team provides tailored therapies such as physiotherapy, occupational therapy, and speech therapy to enhance the quality of life for participants. We are committed to empowering individuals with disabilities, helping them achieve personal goals and independence. With a focus on compassionate and care, we strive to make a positive impact in the lives of those we serve under the NDIS framework.

Transport

At Holy Cross Care Services, we recognise that barriers exist in accessing the community. Our travel support option can help you manage the physical, psychological or social difficulties that you may have with accessing other transport options.

Some of these are:

  • Taking you out to meet your appointments.

  • Helping you to go to market for shopping essential commodities

  • Supporting you with your social, professional and community outings

  • Assisting you with mobility in and out of vehicles

Home Modifications 

We understand the need to have some modifications in your house to ensure that safety is never compromised. Home modifications services are changes to the structure, layout or fittings of a participant’s home so they safely access it and move around comfortably in areas they frequently use.  

We provide support for a wide variety of complex home modifications including, but not limited to:

  • Bathroom modifications 

  • Kitchen modifications

  • Minor modifications

Allied Health and Therapeutic Services

Assistance with Social & Community Participation

Community participation in social gatherings and events is very important, more so for people with disabilities. Participants in the NDIS can benefit from such activities as it also helps them in learning new skills. Our NDIS Social and Community Participation Activities would involve but not restricted to;

  • Cooking classes

  • Dance and music

  • Concert or recreational activities

  • Skill building or personal development

  • Social gatherings

  • Visiting a library, movie or a concert

Coordination of Supports

Getting started with a NDIS plan can be a confusing process. Care Holy Cross Care Services Support Coordination services, we can work with you to connect to the supports that will work for you including;

Supporting participants to understand and implement the funded supports in their plan.

  • Linking participants to the community and mainstream

  • Negotiating service costs with providers and budget participant plan accordingly.

  • Ensuring service agreements are completed in line with the participants’ goals.
